Information we collect

• Contact form fields: name, email and/or phone, preferred contact method, general reason for contacting us, and an optional short message.
• Technical data: standard server logs may include IP address, user agent, and request timestamps for security and rate limiting.

Please do not submit diagnoses, medication lists, detailed symptoms, insurance IDs, dates of birth, or other sensitive medical information through website forms.

How we use information

• To respond to appointment or callback requests
• To answer general questions about hours, location, or services
• To protect the site against spam and abuse

Analytics and advertising

By default this site does not load Meta Pixel, advertising trackers, session replay, or heatmaps. If privacy-conscious analytics are added later, this page will be updated to name the provider and data collected.

Third parties

• Hosting / CDN: may process IP addresses and request metadata to serve pages (for example Cloudflare or another host configured at deploy time).
• Maps links: choosing Directions opens Google Maps or Apple Maps, which have their own privacy policies.
• Optional form webhook: if CONTACT_WEBHOOK_URL is configured, non-message contact fields may be delivered to that endpoint for email/ticketing.

Retention

Form delivery retention depends on the inbox or webhook destination once configured (TODO). Free-text form messages are not written to application logs.
